Instructions for Federal Stafford Loan Master Promissory Note/Federal Parent PLUS Application

Thank you for your interest in the Federal Stafford Loan Program.

To expedite the application process, please follow the directions below:
You have the option to fill out your application in two ways.

  1. Complete the application online, print out and sign the application.
    • It is mandatory to indicate the name of your school, school code (if known), city and state in the upper right-hand corner and to fill in the lender name (Paragon Federal Credit Union) and lender code(812095).
      • Return the completed and signed application to Paragon Federal Credit Union.
  2. Or, print the blank application
    • Complete and sign the application
