Change of Address Form

This form allows you to change the address on your account at Visions Federal Credit Union.

To expedite the application process, please follow the instructions below.

Please provide a copy of the following documents along with your application:

• Driver’s license
• A recent utility bill showing the new address.

Return the completed and signed application by:

• faxing it to (201) 358-6446,
• mailing it to Visions Federal Credit Union, P.O. Box 400, Montvale, NJ 07645-0400,
• or, bringing it in to any branch location

Your application will be processed once we receive your completed and signed application.
